Programs and Services

Case Management - In an effort to focus on the many issues, strengths, and concerns of sickle cell clients, case management is the main category of serve provided by the NELASAF. Case management is a process for assessing the client’s total situation and addressing the needs and problems found in that assessment.

As a part of this process, the client’s strengths and interest are used to improve the overall situation wherever possible. The primary purpose for the use of case management by the NELASAF is to improve the quality of life for the clients served.  As stated, the objectives of this organization coalesce with the second primary purpose of the case management methods, that is to prevent problems from growing worse and costing more to remedy in the future.

 

Services under the Case Management Umbrella:

Assessment of Need / Referral Services

Counseling Services – Individual, family as requested by clients

Medical Assistance – Distribution of Medical Monetary Assistance

Transportation Assistance

Hospital Visitation

Med-Camp

Bereavement Support

Community Education – To be conducted in each parish served on a quarterly basis at minimum.

Know Your Sickle Cell Status Campaign

Educate the Educator
